Family Medicine 2025
What You
Really Need to Match as an IMG

Family Medicine 2025: What You Really Need to Match as an IMG

Matching into Family Medicine (FM) as an international medical graduate (IMG) is achievable—but not automatic. With over 5,300 positions offered in the 2025 Match, FM continues to be one of the most IMG-friendly specialties. But what actually separates the successful applicants from those who don’t make the cut?
Here’s a breakdown of the latest 2025 match data, based on NRMP statistics and outcomes for U.S. and non-U.S. IMGs targeting Family Medicine.

Overall Match Landscape for Internal Medicine

  • Total FM positions offered: 5,375
  • Total applicants who applied FM: 7,337
Total matched IMGs in PGY1 positions: 1427
  • U.S. IMGs matched: 626 out of 1,277 applicants
  • Non-U.S. IMGs matched: 801 out of 2,123 applicants
Bottom line: FM remains a highly accessible specialty for IMGs, with 49% of U.S. IMGs and 38% of non-U.S. IMGs successfully matching in 2025.

U.S IMGs: Charting outcomes from 2024 report of a Successful Match

  • Matched U.S. IMG Profile (2024 Match Report):
  • Step 1 Score: 209
  • Step 2 CK Score: 228
  • Research Experiences: 1.8
  • Publications/Presentations: 2.7
  • Work Experiences: 4.7
  • Volunteer Experiences: 3.8
  • Contiguous Ranks: 8.7
  • Specialties Ranked: 1.3
  • PhD Holders: 0.2%
  • Other Grad Degrees: 22.7%

For U.S. IMGs who matched into Family Medicine in 2024, having a balanced and focused application was key. The average Step 1 score was 209 and Step 2 CK was 228—not very high, but enough when combined with other strengths. These applicants had some research (1.8 experiences and 2.7 publications), but research was not the main factor. What really helped was strong clinical experience (4.7 work experiences), volunteering (3.8 experiences), and ranking enough FM programs (8.7 on average). Also, 22.7% had another graduate degree. The main point: Family Medicine is a great option if you have average scores but good experience and a focused plan.

Non-U.S. IMGs: Charting Outcomes from 2024 season for a Successful Match

Matched Non-U.S. IMG Profile (2024 Match Report):
  • Step 1 Score: 217
  • Step 2 CK Score: 231
  • Research Experiences: 2
  • Publications/Presentations: 3.7
  • Work Experiences: 4.2
  • Volunteer Experiences: 3.4
  • Contiguous Ranks: 5.6
  • Specialties Ranked: 1.3
  • PhD Holders: 1.7%
  • Other Grad Degrees: 24.1%

Non-U.S. IMGs who matched into Family Medicine in 2024 had strong, balanced applications. Their average Step 1 score was 217 and Step 2 CK was 231—good, but not extremely high. What helped most was their overall experience. They had about 4 work experiences, 3 volunteer experiences, and some research (2 research projects and 3–4 publications or presentations). They ranked around 6 FM programs and mostly focused on just one specialty. Around 24% had another graduate degree, and a few (1.7%) had a Ph.D., but these were not required. The main takeaway: having solid scores, good clinical and volunteer work, and a clear focus on Family Medicine gives non-U.S. IMGs a good chance to match.

Comparing Family Medicine to Other IMG-Friendly Specialties

Insight: If your scores are moderate and research is limited, Family Medicine, Pediatrics, and Psychiatry offer solid match potential. For those with a strong academic and research profile, specialties like Pathology or Neurology are viable but more competitive.

Key Takeaways

  • Scores Still Matter: U.S. IMGs should aim for Step 1 > 210 and Step 2 CK > 225. Non-U.S. IMGs should target Step 1 > 215 and Step 2 CK > 230 for a competitive shot at FM.
  • Be Strategic With Your Rank List: The difference between matched and unmatched applicants often comes down to the number of contiguous ranks. Aim for at least 6–8 strong programs.
  • Research Isn’t Everything: For FM, strong clinical, volunteer, and work experience can outweigh a thin research background.
  • Keep It Focused: Ranking multiple specialties doesn't necessarily improve your chances. Most successful applicants ranked only Family Medicine or one additional specialty.

Final Word

Whether you’re a U.S. or non-U.S. IMG, Family Medicine is a great gateway into the U.S. healthcare system. With moderate scores, a focused application, and solid clinical exposure, your chances of matching are real.
Ready to take the next step? Let your application speak louder than your scores. Prep smart, rank wisely, and don’t underestimate the power of a well-rounded profile.
