How Jose Luis, an IMG from Mexico, Matched into Family Medicine!

 
I graduated from medical school in Mexico in 2019, and in 2024, I matched into a Family Medicine residency in the United States as an IMG.

This journey took years of USMLE preparation, US clinical experience (USCE), research, networking, and persistence. It was not easy, but it taught me that matching is about much more than just exam scores.

My journey followed the typical IMG residency match pathway--
  • Completing USMLE Step 1, Step 2 CK, and Step 3
  • Gaining US clinical experience (USCE)

What I learned is simple: no single part gets you matched. A strong IMG residency application is built when all these pieces come together strategically.

USMLE Exam Strategy: What Jose Learned as an IMG

The USMLE exams were one of the toughest parts of my IMG residency journey. My biggest advice? Timing matters. Delays in completing USMLE Step 1, Step 2 CK, or Step 3 can raise concerns about your year of graduation (YOG) during the residency match process.

I also believe USMLE Step 3 is often underrated—it can strengthen your IMG residency application by showing clinical readiness, commitment to the U.S. medical system, and making your profile more competitive for residency programs.

How AAFP Conference Changed Jose’s Match Odds!

One of the best decisions in my IMG residency match journey was attending the AAFP (American Academy of Family Physicians) Annual Conference in Kansas. It gave me the chance to meet program directors, residents, and residency coordinators face-to-face, ask meaningful questions, and build genuine networking connections.

Looking back, attending the AAFP Annual Conference was one of the most valuable parts of my Family Medicine Match journey. For any IMG applying to Family Medicine residency, attending specialty-specific conferences and networking events should be a priority—they can create real opportunities, strong connections, and improve your residency match chances.

What IMG Applicants Need Beyond USMLE Scores?

One of the biggest lessons from my IMG residency match journey is this: there is no single factor that guarantees a Match. Family Medicine residency programs look at your complete IMG residency application, not just your USMLE scores.

A competitive IMG application for Family Medicine typically includes:
  • Strong USMLE Step scores (especially Step 2 CK)
  • Recent US clinical rotations in relevant specialties
  • Research experience (even case reports or poster presentations help)
  • Letters of recommendation from US physicians who know you clinically
  • A compelling personal statement that reflects your story and goals
  • Demonstrated communication skills and patient-centered approach
  • Networking with programs through conferences, emails, and connections
It is the complete package that opens doors — not any one element in isolation.

Can Older IMGs Still Match? 

Answer for 2019 Graduates and Beyond

One of my biggest concerns as a 2019 medical graduate IMG was my year of graduation (YOG). I worried that residency programs would filter me out automatically. But I learned that many IMG-friendly Family Medicine programs look beyond YOG and evaluate the full residency application.

My advice to IMGs: don’t self-reject. Build a strong IMG profile with USMLE scores, USCE, research, LORs, and networking, and apply strategically to programs that truly consider international graduates.

What Family Medicine Residency Programs Look For in IMGs?

Family Medicine is a relationship-driven specialty. Programs select residents who can connect with diverse patients across all ages, backgrounds, and health conditions. For IMGs, this is often a natural strength — and one worth highlighting.
 In my experience, the following qualities as particularly valued by FM programs:
  • Communication skills and cultural humility
  • Patient interaction experience across diverse populations
  • Clinical readiness and documentation skills (EMR proficiency, SOAP notes)
  • Professionalism and teamwork
  • Letters of recommendation that speak to patient care quality
Strong letters of recommendation are especially powerful in Family Medicine because they can paint a picture of who you are with patients — something scores alone cannot capture.

2. Do IMGs need to take Step 3 before applying to Residency?

Step 3 is not required before the Match, but taking it can strengthen your application — especially for IMGs applying to Family Medicine. It signals clinical readiness and sets you apart from applicants who have not yet taken it.

3. How important is in-person networking for IMGs?

Networking was extremely important in my IMG residency match journey. My experience at the AAFP conference showed me that meeting program directors, residents, and coordinators in person can directly impact how your residency application is viewed. Strong IMG networking turns your application from just a file into a real person—and that can make a big difference in the residency match process.

4. What USMLE scores do IMGs need for Family Medicine?

Score requirements vary by program, but most IMG-friendly Family Medicine programs look for Step 1 pass and Step 2 CK scores in the 220–240+ range. Higher scores expand your pool, but programs also weigh rotations, research, and communication skills heavily.

Final Advice for IMGs Applying to Family Medicine

“Connect with people face-to-face. Be remembered. There are thousands of IMGs applying every year. If programs remember you, that connection can open a door that scores alone never will.”
Just Remember one Thing: Match success for IMGs is not about being perfect. It is about being prepared, being strategic, and putting yourself in rooms where opportunities are made.
