Leveraging Home Country Residency and Switching Speciality

​Older International Graduates: their USMLE Journey

USMLE attracts a broad spectrum of healthcare workers and receives applications that are diverse, therefore, need a preparation strategy that could nurture and fit the unique profile.

Older medical graduates are persistently overlooked and get little attention from agencies. Unlike what freshers aim for, a chunk of older graduates seek to switch their speciality and have often migrated from a different country.

To cater to this recurrent element that presents in some profiles, USMLESarthi interviewed two Older International Medical Graduates (IMGs) with a home country experience, who cleared the USMLE and got into their desired residency.

Anagha and Divya, are both older graduates with a home country residency who have unique experiences to offer to our readers. Not only did they get through the exam despite their mature age, Anagha also switched into a different speciality.

Anagha, a 2011 Graduate

Anagha immigrated to the US after being in an advanced stage of her medical career as a neurosurgeon. She is a 2011 medical graduate, who was then pursuing a residency in her home country, India, as a neurosurgeon. It was in her 6th year of her residency that she decided to move to the US and practise in a different speciality. 

She was preparing to get matched into neurology. This looked difficult with the two striking challenges that reflected boldly in her profile.

First, switching speciality and then, switching speciality  as an older graduate.


To surpass these hindrances, Anagha curated her own timeline. 

An early commencement of preparation for the steps acted as a head-start. 

As for Anagha, this was 2 years ahead of her match year. This time span you give to yourself for an advanced preparation buffers the strain that builds towards the end as the exam nears.

This makes sure you have all the documentation in place and enough LORs from the US. Anagha also utilised her time by getting into as many  observerships as she could.

It was around this period that she got into contact with Mr. Pawan, founder of USMLESarthi.

Divya Reddy’s, a 2015 graduate

Divya is a 2015 graduate from India, who joined USMLE Sarthi in pursuit of USMLE. She had done her residency in paediatrics until 2018 and moved to work in the UK before finally setting her feet on USMLE preparation.

The Journey

Divya started her preparation for USMLE towards the end of 2021 and appeared for all the three steps in the following match season of 2022. 

Divya had just given birth, this made her journey a bit more rugged, along with being an older graduate. Despite the apprehensions, she passed through all the three steps, scoring appreciably.

Initially, Divya had absolutely no information about the structure of USMLE, its how-tos and what-tos. USMLESarthi had turned out incredibly for her cousin, who convinced her to get in touch with us. Owing to the initial confusion and pregnancy, her association with us was delayed. She joined USMLE Sarthi in August of 2022.
